Electronic Immediate Discharge Documentation
Electronic Immediate Discharge Documentation(eIDD) is the electronic version of the Patient Discharge Letter and Medication Summary. eIDDs are generated by all medical and non-medical prescribers (including pharmacists who transcribe) and are authorised by clinical pharmacists. After processing, the eIDD is sent to the GP via an electronic intraythat they can access via their practice computer.
This electronic system ('Casenotes: Electronic Discharge Notes') is in place across the Oxford Radcliffe Hospitals, and is accessed via the Clinical Intranet. It is currently used by pharmacists and prescribers working on inpatient wards when discharging patients.
Electronic Immediate Discharge Documentation (eIDD) is being introduced to meet the target of ensuring that GPs receive a patient’s discharge information within 48 hours from when a patient leaves the hospital.
The benefits of eIDD are:
-
for GPs: eIDD is clear, legible and received within 48 hours of discharge
-
for patients: improved clinical pathway
-
for pharmacy: reduction in TTO errors
-
for prescribers: accessible, easy-to-use system.
In short, eIDD provides a single, simple, swift, standard, system that will improve both the accuracy and quality of our discharge information, as well as ensuring that this information is dispatched in a timely and efficient manner. Furthermore, it will streamline the process for clinicians, freeing up their time and minimising risk.
At present, GPs have access to Casenotes via the 'Doctors' Desktop' from their PC. Information that is currently available includes: lab results (bloods and some x-rays), dietetic letters and the current eIDD forms generated from Paediatrics, Geratology and the Chest Medicine.
The benefits from eIDD for GPs are significant:
-
immediate access to a patient's discharge information
-
increased clarity and accuracy of content
-
all mandatory fields completed.
GPs will be contacted to be updated on the progress and future planning of the eIDD project. Training will be available to individual GP practices should they wish to refresh their knowledge of the system in preparation for the enhanced software roll-out.
Staff can access the eLearning modules via the intranet.
