Referring via Choose & Book
Use Choose & Book for outpatient appointments:

Routine referrals
Patients should always be referred via the Choose & Book system. The consultant will assess each referral beforehand and the patient will be seen first by a specialist nurse, who will ensure that all required investigations are done. Results will be reviewed by the consultant, who will decide whether the patient needs a follow-up in the outpatient clinic.

Urgent referrals
All urgent referrals should now go through Choose & Book, rather than being faxed to the department.
Use Choose & Book for 24 hour ambulatory blood pressure monitoring only.

Routine and follow-up
Ambulatory blood pressure monitoring requests should always be referred via the Choose & Book system and patients will be seen in the nurse-led clinic and results reported by consultant.

All urgent referrals should now go through Choose & Book, rather than being faxed to the department.
